Health History Lesson Plan
Objectives- Students will be able to:
- Familiarize student with health history forms and develop knowledge about their value and
how to fill them out.
Materials
Activity (for M-level)
- Start by reading through Worksheet 1 with your student. Take turns reading and make sure that they understand each sentence.
- Apply what you have learned by filling out the Patient Information Form with your student. If they don’t know some of the answers to the questions, have them agree to find out when they go home and then fill out the rest for homework.
Activity (for E-level)
To make the lesson simpler for students with lower literacy, make the following adjustments:
- Start by reading through Worksheet 1 with your student. Take turns reading and make sure that they understand each sentence.
- Apply what you have learned by filling out the Patient Information Form with your student. Pay special attention to spelling and pronunciation of words. Practice looking up any words they don’t know in a dictionary. If they don’t know some of the answers to the questions, have them agree to find out when they go home and then fill out the rest for homework.
Activity (for D-level)
To make the lesson simpler for students with higher literacy, make the following adjustments:
- Start by reading through Worksheet 1 with your student. Take turns reading and make sure
that they understand each sentence. - Apply what you having your student fill out the Patient Information Form, explaining questions that are confusing. If they don’t know some of the answers to the questions, have them agree to find out when they go home and then fill out the rest for homework.
